If you get this error, it is often because you have configured Apache with modules that weren't actually installed. If you are getting this error it is usually caused by having more than 5 keys in your ".ssh" directory. It is a bit of a bug and this is how it manifests itself.
You will find at this point that you are not given any chance to enter a password, or if you are using key based auth that the same thing happens. You'll also find that this is happening with ALLservers you try connecting to.

The most common solution is to use the /etc/postfix/header_checks but this is a big problem.
Why is header_checks a problem? Because it does it to all mail whether incoming or outgoing and whether authenticated or not. I experienced this with CPanel's Exim after an auto-update (a Google of this error produces lots of complaints with few clear solutions). In this case I'll put the solution at the top.
